I was applied for SVMCM but it was reject due to incorrect order of income proof certificate upload. When I try to rectify and upload the correct one, suddenly saw the other small form named Self Declaration from but i can not understand that what is written in the blank filed ( Now I am going to present my corrected document(s) for verification by the
Authority on __________________).Please help me to complete the form and re-submit again.
1st suggestion, Don’t Share your Application Public..!! When you will submit the rectified/corrected document.. you have to fill the self declaration and upload it.. The date you are Uploading… will be in the____(25-11-2023)______ blank field… The self declaration also included your Signature..!!
